Scott Paxton, DO dr paxton

Dr. Paxton is Board Certified in Family Medicine and Hospice and Palliative Medicine. He received his Osteopathic Degree from the University of Osteopathic

Medicine and Health Sciences and completed his Residency at Garden City Hospital.

Before joining Go Docs Go, Dr. Paxton was the Palliative Medical Director at Great Lakes Caring, where he worked for 5 years. Prior to Great Lakes, he was the Associate Medical Director at Arbor Hospice and also worked as a Family Physician for 10 years. He is an assistant clinical professor at the Pikeville School of Osteopathic Medicine. Dr. Paxton has been a member of the American and Michigan Osteopathic Association for almost 20 years and is also a member of the American Academy of Hospice and Palliative Medicine.

Dr. Paxton enjoys house calls because in his opinion, primary care and non-urgent supportive care are delivered most effectively in the patient’s home.

Dr. Paxton is the president of an asthma camp that provides opportunities for underprivileged youth with asthma to attend summer camp. He enjoys outdoor sports with his two sons, including biking and skiing.

Raymond Hansen, MD

Dr. Hansen is Board Certified in Internal Medicine.  He received his Medical Degree from Michigan State University and went through his Residency program at Michigan State University.

Before joining Go Docs Go, Dr. Hansen was the Chief of Staff of St. Lawrence Hospital and served as the Chairman of the Department of Medicine on two occasions.  He is a clinical professor at the Michigan State University.  Dr. Hansen has spent many years with Mid-Michigan Physician group as he is the former CEO and President of MMP.

Dr. Hansen vast clinical knowledge, experience and exceptional bedside manner create a memorable house call experience.

Dr. Hansen and his wife, Dona, enjoy their 5 children, grandchildren, golf, traveling and gourmet cooking.

Amelia Fajardo, MDPic - Fajardo

Dr. Amelia Fajardo has been in family practice since 1962.  She specialized in obstetrics and gynecology from 1972 – 2008.  In 2011 she returned to family practice. She has a passion for providing the best care possible for her patients.

Dr. Fajardo received her Medical Degree from Manila Central University in the Philippines.  She completed her residency at St. Luke’s Hospital in St. Paul MN.  She completed and received her specialty training in O.B. GYN. at St. Barnabas Hospital in Minneapolis MN.

During her long career she has received several prestigious awards.  The American Medical Association presented her with “The Best OB of the Year Award” in 1995.   The MPNA presented her with “The Best OB of the Year Award” in 2004.  She was also awarded “The Michigan Governors Award of Distinction” in 2008 for a distinguished 45+year career.

In her spare time she enjoys traveling and playing tennis.

Patty Hopkins, FNP-BC

Patty is a board certified Family Nurse Practitioner through the American Nurse Credentialing Center.  She enjoyed spending time in Sault Ste. Marie receiving her bachelor’s degree in Nursing from Lake Superior State University.  Patty completed her Master’s of Science in Nursing from Michigan State University.

Before joining Go Docs Go, Patty worked for many years as a Registered Nurse and Family Nurse Practitioner at St. Joseph Mercy Hospital.

Patty develops a relationship with each patient.  She takes pride in establishing rapport with the patient and the patient’s family.

Patty is a nature enthusiast, avid runner, walker and biker.  She grew up in Travers City, Michigan on a dairy farm.  Patty and her husband love watching their two girls compete in cross-country meets.

Elaine Philipson, ACNP-BCPic - Philipson 2

Elaine Philipson is a Board Certified Acute Care Nurse Practitioner through the American Nurse Credentialing Center.  She received her Master’s Degree from the University of Michigan.

Before joining Go Docs Go, Elaine worked and continues to work as a Survival Flight Nurse for the University of Michigan.  She is a part of emergency and critical care transport team for ground, helicopter and airplane assignments.  She received the National Medical Crew Member of the Year Award in 2004.

Elaine enjoys looking at the patient holistically.  She takes pride in caring for the patient’s physical, emotional and familial needs.

Elaine was born in England.  She has been in the States for over 20 years and married to her husband for 24 years.  They have one teenage son.  Elaine and her husband enjoy traveling and their blind border collie.

Ann Michelle Webb, CFNP-BC

Ann Michelle Webb is a Board Certified Family Nurse Practitioner through the American Nurse Credentialing Center.  She received her Bachelor’s Degree from the College of St. Elizabeth and her Master’s Degree from the University of Indianapolis.

Before joining Go Docs Go, Ann Michelle worked in both in-patient and out-patient settings in the Lafayette area. Her primary focus has been in general practice and family medicine.

Ann Michelle approaches each housecall with a listening ear.  She focuses on patient-centered holistic care.

Ann Michelle enjoys riding motor cycles in her free time as she is a Harley Davidson guru.  She also enjoys spending time with her two teenagers who play sports year round.

 

Roslyn Williams, FNP-BCPic - Williams

Roslyn Williams is a Board Certified Family Nurse Practitioner.  She received her Master’s Degree from South University, her Bachelor’s Degree from Oakland University and her Associate Degree in Nursing from Henry Ford.

Before joining Go Docs Go, Roslyn worked in home care for 5 years as well as the Providence Park Emergency Room Department.  Also, she has experience in long term care facilities during her years as a nurse.

Roslyn likes providing care to geriatric patients because she loves learning about her patient’s history and stories.  She enjoys making housecalls because she develops a compassionate clinician-patient relationship.  During each housecall she provides a listening ear.

Roslyn enjoys spending time with her husband and two children.  In her free time she plans to start mentoring inner-city school children.

Taris Gonzalez, ANP-BC

Taris graduated from Andrews University with her Masters of Science in Nursing as well as her Bachelors of Science in Nursing.  She is a Board Certified Adult Nurse Practitioner.  Taris has been practicing as a Nurse Practitioner for over 13 years.

Before joining Go Docs Go, Taris worked in primary care at Community Health Network.  She has also had experience providing care in the patient’s home.  Taris is excited to continue providing primary care in the home setting.

Taris loves to share her clinical knowledge by providing education to her patients.  She believes in providing patient centered care at each visit.

Taris loves to spend time with her three children and is an animal lover.  In her spare time she enjoys riding motorcycles.

Miriana Popadich, FNP-BCPic - Popadich

Miriana Popadich is a Board Certified Family Nurse Practitioner.  She received her Master’s Degree from Michigan State University.  Miriana has been a Nurse for almost 20 years and a Nurse Practitioner for over 5 years.

Before joining Go Docs Go, Miriana worked and continues to work in-patient and out-patient Neurosurgery, Acute Pain Management and Cardiology practices.

Miriana is pleased to be able to provide Primary Care to her community.  She is eager bridge her background in Cardiology and Acute Pain Management with Primary Care.

Miriana was born in Romania and is fluent in Romanian and Serbian languages.  She has lived in the United States for 35 years.  She enjoys spending time with her husband of 20 years and their two boys.  In her free time Miriana and her family enjoy skiing and traveling.

Judy Jerome, GNP-BCPic - Jerome

Judy Jerome is a Board Certified Geriatric Nurse Practitioner.  She received her Associate’s Degree from Lansing Community College, her Bachelor’s Degree from University of Michigan and her Master’s Degree from Michigan State University.  She has been a Nurse Practitioner for over 20 years.

Before joining Go Docs Go, Judy worked in Internal Medicine and Geriatric Care at Ostego Memorial Hospital in Gaylord for almost 10 years.  She spent the majority of her career caring for geriatric patients in Northern Michigan.

Judy believes in providing compassionate care to her patients.  She enjoys educating her patients and those that assist in care giving.  Her kind nature is evident in all aspects of her care planning.

Judy loves to spend time in Northern Michigan, working on her family’s farm and raising her chickens.  She also enjoys spending time with her family and nephews.

Jennifer Paulson, ANP-BCPic - Parker

Jennifer Parker is a Board Certified Adult Nurse Practitioner.  She received her Bachelor’s Degree from Sienna Heights University and her Master’s Degree from University of Toledo.  She has been in the nursing field for 17 years.

Before joining Go Docs Go, Jennifer worked with geriatrics for the majority of her career.  She has worked in primary care, skilled nursing facilities and as a hospitalist.  These experiences give her an advantage caring for acutely ill patients in their homes.

Jennifer’s true passion is geriatric care.  Her goal is to optimize her patients’ quality of life and keep them happy and in their own home. She has lived in Lenawee County her entire life and looks forward to caring for her community.

Jennifer loves to garden and spend time with her husband and 6 children.  She is a staff member for the Lenawee County Young Marines and a Board Member for the Daily Bread.

Kyle Ploehn, PA-CPic - Ploehn

Kyle Ploehn is a Board Certified Physician Assistant.  He received his Bachelor’s Degree from the University of Michigan and his Master’s Degree from Western Michigan University.  Kyle has been a Physician Assistant for over 13 years.  Between his schooling, Kyle proudly served our Country for 6 years in the US Air Force spending time in the Middle East during Operation Desert Shield/Storm and Operation Provide Comfort.

Before joining Go Docs Go, Kyle worked in family medicine and worked with ENT, pulmonary and sleep study specialists.  He comes to Go Docs Go with 2 years of experience in in-home primary care.  Kyle has a strong background in fitness and exercise, orthopedics and osteopathic medicine.

Kyle is truly enthusiastic about house calls.  He believes house call medicine puts the patient first and allows for the unique opportunity to provide medical care in the comforts of a patient’s home.  Kyle enjoys spending quality time with his patients and learns invaluable information about each patient allowing him to tailor a personal plan of care to each individual.

Kyle puts the “fanatic” in being a fan of the University of Michigan, particularly Wolverine football and hockey.  He is the proud father to two teenagers, a daughter and son, who excel in academics and athletics.  He enjoys exercising, skiing, traveling and having a productive garden.

 Kelly Stovall, ACNP-BCStovall, Kelly

Kelly is a Board Certified Acute Care Nurse Practitioner.  She received her Bachelor’s Degree from Kaplan University and her Master’s Degree from Wayne State University.  Kelly has been in the nursing field for over 14 years.

Before Go Docs Go, Kelly worked in Critical Care Services and Metro Detroit Emergency Department as a Nurse Practitioner.  Prior to becoming a Nurse Practitioner, she worked in in Emergency Services as a clinical nurse and as a Emergency Department Educator and Stroke Coordinator. Kelly also has experience in Urgent Care and the Trauma-Burn Unit.

Kelly brings a a sense of happiness to a house call because for me, the person I am seeing is in their own environment.  She believes this is the best way to see the person as whole.  In a clinic or hospital setting, the person is not in their own environment and often do not have any sense of control; lighting, sounds, etc..  By being able to see a person in their own environment, it allows her to see them and how they are doing.  This is a rewarding aspect to housecalls.

Kelly is a a mother to two grown sons (twins) who are both active duty Army and have both been to Afghanistan, and a daughter who is in elementary school.  Additionally, she is married and has a Step-daughter, and a Step-son and Step-Daughter-in-law and a granddaughter.  She enjoys all kinds of music, especially classical, reading a lot, spending time with her family, and if time allows, she likes to sew.

Erin Griffith AGNP-BCerin

Erin is a Certified Adult/Geriatric Primary Care Nurse Practitioner. She received her nursing degree from Baker College and received her Master’s Degree from South University.

Before joining Go Docs Go, Erin worked as a Nurse Practitioner for Clinton County Medical Center. Here she cared for patients of all ages in a Primary Care setting. Erin worked as an ER nurse at Sparrow hospital prior to becoming a Nurse Practitioner.

Erin strives to develop therapeutic relationships with patients, families, and other professionals. She aims to streamline her geriatric patients’ medications, eliminating unnecessary medications, while maintaining a balance of overall health with quality of life. She has a passion for the geriatric population and promotes safety, independence, and dignity.

Erin is married to her High School sweetheart and has two children. She also has a bulldog named Bo. In her spare time, she enjoys road trips, watching movies, and spending time with family.

Codi Schrauben ANP-BC

Codi Schrauben is a Board Certified Adult Primary Care Nurse Practitioner. She received her Bachelor’s Degree from Oakland University and her Master’s Degree from Wayne State University.

Before joining Go Docs Go, Codi worked as an Adult Nurse Practitioner & Nurse Educator at Optimal Care, Inc. Here she provided care and education to homebound patients. She also worked as an Adult Nurse Practitioner at West Bloomfield Internal Medicine where she treated a variety of chronic and acute illnesses. Codi worked as a staff nurse at Detroit Medical Center, Harper Hospital prior to becoming a Nurse Practitioner.

Codi enjoys developing therapeutic relationships with patients, families, and other professionals. She is passionate about this collaboration to develop a mutually beneficial plan of care, which promotes healthy lifestyles and disease prevention.

Codi is ecstatic to have recently moved with her husband and two daughters back to her home town, allowing her more time with family and friends. In her spare time, she enjoys camping, road trips, reading, and running.

 

Jenny White FNP-BCjenny white

Jennifer White is a Board Certified Family Nurse Practitioner. She received her Bachelor’s Degree from the University of Toledo and her Master’s Degree from Wright State University in Dayton, Ohio.

Jennifer has been a Nurse Practitioner for almost 20 years. Before joining Go Docs Go, Jennifer worked and continues to work at Michigan Pain Consultants in the Grand Rapids, MI area. She has been working in pain management since 2002. Prior to becoming a Nurse Practitioner, Jenny worked as a RN in home health and hospice agencies, schools, and has worked at Cincinnati Children’s Hospital.

Jennifer is enjoying doing home visits again. It is a more relaxing and rewarding experience for her and the patient. She feels it is the only way you can really get to know and help your patients and families.

Jennifer is married to Tom who is a teacher and football coach. They have three children who still keep them busy even though they have an empty nest. She enjoys outdoor activities such as running and hiking, and is also involved with the Polish Falcons. She and her husband have a second home up north in Bliss, MI where they like to spend their spare time.

Amber Crowe AGACNP-BCamber

Amber Crowe is a Board Certified Adult-Gerontology Acute Care Nurse Practitioner. She received her Bachelor’s Degree from Wayne State University and her Master’s Degree from Madonna University.

Before joining Go Docs Go, Amber was a Hospice and Palliative Care Nurse Practitioner with Great Lakes Caring. Prior to becoming a Nurse Practitioner, Amber was a Registered Nurse for 13 years. Her experience as a Registered Nurse was at St. John’s Providence Hospital, with the majority of it in the Medical Intensive Care Unit.

Amber enjoys seeing patients in their homes because it truly becomes a partnership and they let her into their life.

In her spare time, Amber likes to spend time with her family and granddaughter. Her hobbies include scrapbooking and boating.
